I know this has been asked in the past, but I cannot find a windows 10 and e-sword 12.1 answer.  I uninstalled and reinstalled the program, but every time I open the program I get a popup that says, "Error Opening topic notes database".  I say "ok" and it opens fine after that.  Any tips to get this to stop?

Try doing another uninstall of the program, after the uninstall completes check your documents folder and look for a folder inside it labeled e-Sword.

When e-Sword installs on a Windows PC, two folders are installed one in the programs folder and one in the documents folder.

The e-Sword folder in the Documents folder contains several small files needed for e-Sword to function correctly. These small files are used with the three editors available in e-Sword which are the Topic, Journal and Note editors. The folder is also the location where your e-Sword Bookmarks list is stored.

The three files needed for the editors are journal.jnlx for the journal editor, study.notx the notes editor and topic.topx for the topics editor.

If after another uninstall the e-Sword folder in the Documents folder is still present, delete the folder manually. You might also want to check the programs folder and see if there is anything left behind after e-Sword uninstalls.

I followed this and it fixed the error message, but now all the topic notes I've had from years past are gone.

Did you backup the files before you deleted them? If you have a backup you can copy it back and you will have your files restored.
