To use the dictionary appendices with the commentary, select the dictionary from the e-Sword dictionary tab. When you click on an Appendix in the e-Sword commentary, the appropriate appendix will appear in this dictionary.

Bullinger was a hyper-dispensationalist and remains controversial, but ironically, his system does not usually shine through noticeably in the work. When Bullinger died in 1913, the Companion Bible was completed only up to the middle of John. His followers finished the work with comments from his prolific writings elsewhere. The name comes from the fact that a wide margin is left to be a companion to the text, and the work is intended to be the Bible reader’s companion.

The Companion Bible is popular among Christians who want to understand God’s Word in the trusted and familiar language of the King James Text. The Companion Bible is sometimes touted by proponents of the KJV as a free and compelling Study Bible that remains immune to the trends of modern Study Bibles and translations.

Extensive marginal notes (presented as as a commentary in e-Sword format) with appropriate Hebrew and Greek analysis provide alternate translations of critical passages and call attention to other relevant texts.

Detailed outlines focus on the context and significance of each book while emphasizing how each part fits into the whole.

Helps include: 198 appendixes of Hebrew and Greek words and their use, including Figures of Speech, charts, parallel passages, maps, lists of proper names, calendars, and timelines., keyed to the marginal notes, provide additional information on genealogies, synonyms, historical and a wide range of other important topics, A “Must” for the Serious Bible Student.

Thanks for getting back to me. I noticed so far one more thing, that seemed to be the more prevalent issues i noticed throughout the notes : 

Other mistakes are omissions of notations: When referred to a Companion Bible Appendix ; For example :
the Companion Bible E-sword version  note for John 3:16 is this - John 3:16
loved. Greek agapao. App-135. A word characteristic of this Gospel. See p. 1511. So App-135 is the abbreviation for  Appendix 135 . What the Appendix  note ; looks like in the printed version is this –   Ap. 135. I. 1. So what’s missing from the e-sword version is the  ”   I. 1.  ”   which tells you where in that particular appendix to look.  That is not very convenient to be missing that.

This is fully described and is all that is said; thank you very much!  Let me encourage those of you who love it, to do a search on ginsbergs full notes; I found them encouraging and excellent: they were downloadable in several formats for free from legitimate download sites.

Hello. This module says that is for 9x and 10x, but e-sword not recognize it any way.

yeah, i got the same problem, i have reinstalled it twice, but no result, still can not find the cbnotes on the esword

This is an application.  It can be Downloaded to Run, Save or Both. Choose one of the options and follow the Install format to load the program. Open e-Sword and go to Commentaries, then open CBNOTES. Search for the Help comments on the Download page and it explains the process.  Hope this helps.

The update made during December 2021 now provides the following download options: (1) ew_bullinger_companion.dcti in HTML format, and (2) ew_bullinger_companion_bible_notes.cmti in HTML format, (3) Bullinger-Companion-Bible-Notes-Appendices-Graphics.exe This third option is applicable for e-Sword version 9x - 10x and later; this third option installs the following files into C:\Program Files (x86)\e-Sword\ 
(a) E.W. Bullinger Companion Bible Appendix Maps.mapx and (B) E.W. Bullinger Companion.dctx and © E.W. Bullinger Companion Bible Notes.cmtx
If you are running e-Sword version 11 and later, copy the ew_bullinger_companion.dcti and ew_bullinger_companion_bible_notes.cmti into C:\Program Files (x86)\e-Sword\ and then you can use File Explorer and delete the E.W. Bullinger Companion.dctx and delete the E.W. Bullinger Companion Bible Notes.cmtx which are for an earlier version of e-Sword.
 
Restart e-Sword to see the results. When you look in the Dictionary option, for example Appendix 165, then move your pointer to the Graphics Viewer to see the pictures, provided from E.W. Bullinger Companion Bible Appendix Maps.mapx. Refer to e-Sword User's Guide, which you call up with function key "F1". Read about the Graphics Viewer on pages 218-221.  
 
E-Sword modules are loaded alphabetically, on the file name. As such, a .dcti file will load in e-Sword before a .dctx file with the same name. e-Sword will only load one instance of a module with the same abbreviation as described in the Details table of the database. Because the .cmtx and .dctx are loaded after the .cmti and .dcti files, only the .cmtx and .dctx files are reported when you look at the list of names inside e-Sword. If you are using the e-Sword version 11 and later, after you delete the .cmtx and .dctx files. Then e-Sword will report the presence of the .cmti and .dcti files. Look in the Commentary view, for both the Verse, Chapter, and Book Notes. Look in the Dictionary view, and open the Dictionary Topics icon to see the topics listed.
 
Rick Meyers says that by upgrading to have everything in the .xxxi format, this is a really good idea since the Unicode HTML modules are the only ones Rick Meyers, the designer of e-Sword, personally supports. The previous files, written with Rich Text (used in e-Sword 9, and later), is not portable between operating systems, and modules made for Rich Text are limited to the Microsoft PC. HTML modules are supported all the way back to e-Sword 11, so this probably covers 99% of the users still using e-Sword.
 
HINT: The Graphics file E.W. Bullinger Companion Bible Appendix Maps.mapx is needed to provide the pictures applicable to references in the dictionary. You can see the contents of the graphics file with the Graphics Viewer (icon next to the Reference Library icon), also accessible on Tools, Graphics Viewer. Refer to Chapter 20 in the e-Sword User's Guide, copyright © 2014, pages 217-221.  
 
NOTE: Since 2016-06-17 Rick Meyers provided a "Download" "commentaries", option to download The Companion Bible (E.W. Bullinger). This will download  bullinger.cmtx and companion_bible_appendix.refx Restart e-Sword to see these new additions. However, this Commentary is missing the pictures and diagrams of the appendices 1 to 198. I have reported this to Rick Meyers.

Yea be careful using this as there are a lot of typos . Some cross references are wrong. And appendix numbers are missing. 
Currently the only good copy of the Companion Bible by Bullinger, is the Printed versions. Not electronic. Its good for a quick glance.
But be sure to follow up with the printed coy.
